OUTREACH SUPPORT
Sometimes known as domiciliary care or floating support, outreach support provides flexible and tailored support for people of all ages. Outreach support is for individuals who live in a single tenancy, group living setting, or at home with parents, but requires some additional individualised support based on their need. Support can range from 3 hours a session to 24/7 daily support. Each individual is provided with outcome-focused support during the day and/or night in line with their assessed need.
At Ace24 Healthcare we provide a number of different types of outreach support however, the aim of all of them is to help people gain or maintain their independence in the community. This could include support to go out in the community for shopping, or a day trip out.
All our outreach activities are focussed on achieving positive outcomes for the people we support, including greater independence, confidence and self-esteem.
